Requirements for Corporate / CSI Donors
To complete a corporate donation Section 18A certification, the following documentation is required from the donor organisation:
Company registration number (CIPC) and registered name
Tax reference number
Authorised signatory details for the pledge or donation agreement
Purchase order (PO) number if required by your internal procurement process
Banking details confirmation (for EFT transfer reference matching)
Completed Donor Pledge Form (provided by Sound of Hope Initiative on request)
CSI application form (if the donor requires a formal application process — Sound of Hope will complete this on request)
Requirements for Individual Donors
Individual donors contributing R500 or more are eligible for a Section 18A tax certificate. The following is required:
Full legal name (as it appears on your ID document)
South African ID number or passport number (for non-SA residents)
Email address for receipt delivery
Postal address (optional, for physical certificate if requested)
Proof of payment (EFT confirmation or donation portal receipt
For donations made via the campaign’s online donation portal (PayFast/GivenGain/Backabuddy), these details are captured automatically at the point of donation.
Requirements for Grant-Making Bodies
For grant applications to the National Lottery Commission, Atterbury Trust, Oppenheimer Memorial Trust, and similar bodies, the following documentation is available from the Sound of Hope Initiative on request:
PBO registration certificate and Section 18A approval letter
NPO registration certificate
Audited financial statements (latest available)
Board/Trustee resolution authorising the grant application
Bank confirmation letter (dated within 3 months)
Project plan and budget (this document serves as the foundation)
CVs of key personnel: Campaign Director and Clinical Lead
Letter of Support from Prof. Mashudu Tshifularo on UP/SBAH letterhead
Monitoring and Evaluation framework (KPIs, measurement tools, reporting schedule)

How to donate
Pledge Agreement
For corporate CSI donations, a formal Pledge Agreement can be issued.
The pledge: secures your sponsorship tier, confirms the patient allocation, and enables procurement of the device before full payment is received.
